We will provide an itemized invoice at the end of the exam which you may submit yourself, but reimbursement is not guaranteed.
Please contact your insurance provider to determine your out-of-network benefits as we do not have access to this information. See claim forms below for the most common vision insurances.
Medicare patients will not be able to submit for reimbursement (see below).

MEDICARE PATIENTS:
The optometrist(s) at this office have elected to opt-out of Medicare, as of March 1st, 2017, as permitted by Section 1802(b) of the Social Security act. If you have Medicare, and are seen for eye care at this office, you will be billed the usual & customary rates. By consenting to treatment/services at this office, you agree that you shall not submit a claim for payment to Medicare for our services, nor shall we submit a claim or bill to Medicare, even if such services would otherwise be covered by Medicare. You will be fully responsible for the payments of all services rendered on the date of service, through private insurance or otherwise.
Medicare patients acknowledge that they understand & will comply with our out-out policy by signing our consent form.
