Out of Network Insurance

Let us submit a claim to your vision insurance for you!

While we do not directly participate with vision or medical insurance plans, we are happy to assist you in utilizing your vision plan out-of-network benefits.


We have partnered with Anagram to submit claims on your behalf to most vision insurance plans.


How it works:

  • When scheduling your exam, please provide your vision insurance information including: Full Name, Date of Birth, Plan ID or Last 4 SSN, Insured Full Name & Date of Birth
  • We will review your out-of-network benefits and provide an estimated reimbursement amount prior to your visit. 
  • At the completion of your exam, we will submit a claim on your behalf. 
  • Reimbursement is typically sent directly to your address by your insurance provider within approximately 2–4 weeks.

 • VSP • EyeMed • Spectera • Davis Vision • Superior Vision • Aetna Vision                                     • Blue View Vision • Cigna Vision


If your insurance plan is not eligible for submission through our system, you will be provided with an itemized receipt at the end of your visit, which you may submit independently.


Important Notes

  • We cannot guarantee reimbursement, as all claims are subject to your individual insurance plan benefits. 
  • If your claim is denied, please contact your insurance provider directly for further assistance (we cannot contact your insurance on your behalf). 
  • If you prefer to submit a claim yourself, see provided claim forms below.
  • Payment is due in full at the time services are rendered. 
  • Professional services are non-refundable. 
  • We accept credit cards, cash, and checks. 
  • Medicare patients will not be able to submit for reimbursement (see below).


GLASSES & CONTACT LENS MATERIALS:

We do not have access to pricing or benefits for materials (glasses or contact lenses) purchased through Costco Optical.
For questions regarding those benefits, please contact Costco Optical directly at (941) 554-2814.


MEDICARE PATIENTS: 

The optometrist(s) at this office have elected to opt-out of Medicare, as of March 1st, 2017, as permitted by Section 1802(b) of the Social Security act. If you have Medicare, and are seen for eye care at this office, you will be billed the usual & customary rates. By consenting to treatment/services at this office, you agree that you shall not submit a claim for payment to Medicare for our services, nor shall we submit a claim or bill to Medicare, even if such services would otherwise be covered by Medicare. You will be fully responsible for the payments of all services rendered on the date of service, through private insurance or otherwise.

Medicare patients acknowledge that they understand & will comply with our out-out policy by signing our consent form.
